Igloo Wellness Centre Limited (“we”, “us”, “our”) would like your consent to use your image, video and/or words in our marketing and content. This form is separate from our Health, Safety & Participation Waiver and our Pre-Activity Health Questionnaire. You can take part in all of our activities without giving any of the consents below.
Each consent below is separate. Please tick only those you are happy to give. You can change your mind at any time — see section 4.

2. Your Consents

2.1 Social Media
☐ I consent to Igloo Wellness Centre using photographs and/or video in which I am identifiable on its social media channels, including Instagram, TikTok, Facebook and YouTube.
2.2 Website
☐ I consent to Igloo Wellness Centre using photographs and/or video in which I am identifiable on its website.
2.3 Print and Offline Marketing
☐ I consent to Igloo Wellness Centre using photographs and/or video in which I am identifiable in print and offline marketing, for example brochures, flyers, advertisements and posters in the premises.
2.4 Testimonials and Quotes
☐ I consent to Igloo Wellness Centre using my name and any testimonial or quote I provide in its marketing.
2.5 Ambassador, Influencer and Partner Content
☐ I consent to Igloo Wellness Centre allowing its ambassadors, influencers and content partners to use photographs and/or video in which I am identifiable in their own marketing of Igloo Wellness Centre.
2.6 Marketing Emails and Messages

3. How We Will Use Your Content
Where you have given consent above, you agree that:
- we may use the relevant photographs, video, name, testimonial or quote in the relevant channels for the purposes of promoting Igloo Wellness Centre;
- we may edit, crop or include the content in collages, montages and similar formats, provided your image is not altered in a way that would be misleading or harmful to you;
- we are not required to seek your further approval before each individual use;
- we will not use your content in any way that is unlawful, defamatory or likely to bring you into disrepute.
Once content has been published, we may not be able to recall it from third parties or from devices on which it has been saved or shared. This is explained further in section 4.
4. Withdrawing Your Consent
You can withdraw any of the consents above at any time by emailing us at [insert email].
On withdrawal, we will take reasonable steps to remove your identifiable content from materials under our control going forward. We may not be able to remove content:
- from third-party platforms once it has been published, although we will make reasonable efforts;
- from materials that have already been printed and distributed;
- from cached, screen-captured or downloaded copies held by third parties.
Withdrawing consent does not affect the lawfulness of any use that took place before you withdrew it.
5. Data Protection
Personal data collected through this form is processed in accordance with our Privacy Notice. The lawful basis for using your identifiable content for marketing purposes is your consent under Article 6(1)(a) UK GDPR.
